PARTICIPANT WAIVER AND RELEASE OF LIABILITY
In consideration of being allowed to participate in the Spark Youth Development 2026
Basketball Practice Sessions, I acknowledge and agree to the
following:
1. Behavior
I will ensure that my child(ren) understand the importance of
respecting the mosque property, volunteers, staff, and other
participants.
2. Damages
I acknowledge that I am financially responsible for any damages
caused by my child(ren) to the mosque building or property during
the event.
3. Emergency Contact
In case of an emergency, I can be reached using the contact
information shared earlier.
4. Medical Emergency
I authorize the event organizers to seek appropriate medical care
for my child(ren) in case of an emergency.
5. Allergies/Medical Conditions
I agree to disclose any allergies or medical conditions that may
affect my child(ren)'s participation in the program.
6. Liability Waiver
I release AlHuda Foundation, its staff, and volunteers from
liability for any injuries or accidents that may occur during the
event.
